By completing and submitting the form below you agree and understand the following statements:

    • I authorize the amount of my purchase to be deducted by Cleveland Clinic until paid in full.

    • I am financially responsible for the full amount of my purchase if Cleveland Clinic does not deduct the full amount from my pay.

    • Once my purchase is submitted to Cleveland Clinic for deduction from my paycheck my order is exchange only.

    • I can not cancel my deductions or receive a cash refund for my purchase, even if an item I order can not be delivered.

    • My order will not ship until my employment can be verified at the end of the online payroll deduction sale.
